The parish Health & Wellness Apostolate will host a Flu Shot Clinic for parishioners ages 12 months and older. Assistance with insurance filing will be available.
What to Bring:
• Download and complete the Vaccine Consent Form in advance (available HERE).
• Insurance card (front and back) and photo ID if you intend to file an insurance claim.
Important Notes:
• Medicare/Medicaid may not cover vaccinations administered at our clinic, but supplemental insurance policies are likely to provide coverage.
• After filing, if insurance does not cover the cost of a vaccination, The Little Clinic will mail an invoice to the participant.
• Payment methods: credit card, check, or cash for the exact amount (please note we cannot make change for cash).
